THE NFL AND MOANALUA FOOTBALL PRESENTS:
THE THIRD ANNUAL
JUNIOR PLAYER DEVELOPMENT CAMP
What: Full contact, full gear, week long football camp. Teaching and learning of
football skills.
Where: At Moanalua High School Football stadium.
When: Monday June 12th 2006 to Saturday June 17 2006
M-Fri 4 - 8:30pm and Sat 9am to 1:30pm
Who: If you are 12 to 14 years of age you are qualified - NO weight limit.
You must have the medical wavier/physical information complete to be eligible
For participation. Bring a copy of a birth certificate for proof of age.
How to sign up: Download the waiver form on this site and the medical clearance
form. The waiver form should be completed and signed by a parent or guardian.
The medical form must be signed by a doctor that clears the camper to
participate in physical activity. (If the camper has a physical form from school or
participation in a community league sport he/she can use that form to participate
in our camp if the form is within 1 year old). Once the forms are complete they
can be turned in at the Moanalua High School main office and it will be placed in
Coach Martinez's box. The first 150 campers to turn in completed forms by June
8th 2005 will be admitted into camp.
Registration packets: First come first serve, we have gear for 150 players MAX.
Packets Can be picked up from the Moanalua High School's main office. Or the
required forms can be downloaded from the Moanalua High school football
website under the link to" Summer Camp Info" at www.moanaluafootball.com
Each participant receives: Free NFL practice jersey, Free NFL Ball, Free NFL bag,
Free quality coaching and a ton of fun!!! Cost = FREE!!
NOTE: The forms include the Waiver Form, and the Medical form with a current physical signed by a physician and proof of age.
